For Referring Doctors
Charlotte Dental Implant Center accepts referrals from general dentists, periodontists, orthodontists, and other dental specialists throughout the Charlotte metro area. To refer a patient, call (704) 375-4252 or fax the referral form and patient records to (704) 364-8353. We provide detailed post-treatment reports back to the referring doctor.
